We are proud to provide a state-of-the-art facility for the highest quality spine care available. After 19 years at the Heater Rd. location we have expanded and moved to a larger facility at 37 Spencer Street, down from the CCBA in Lebanon, NH.
Appointments
If you would like to make an appointment, please contact our office by phone or email. We are open 5 days a week, Monday - Friday. Our office hours are 7:30am until 11:00am, then from 2:00pm until 4:30pm. To save you time we recommend that you download our new patient forms and fill them out prior to your visit. The three forms that we require are the Patient History form, the Financial Policy, and the Practice Requirement form.
Financial
We accept most health insurances (BCBS, Cigna, Harvard Pilgrim and Medicare to name a few) and will submit claims electronically to them. We highly recommend that you check with your insurance company to see if we are on their preferred provider list and so that you understand your financial responsibility. Co-payments and deductibles are due at the time of service. Forms of payment accepted by the office are check, cash, Mastercard and Visa.
Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient.
